• • • Our program will begin in July 2024 and evaluate based on care coordination, patient quality of life, caregiver burden, utilization, and long-term nursing home stay rates.

• Who is Eligible?
  ✔️ Traditional Medicare beneficiaries with a confirmed dementia diagnosis
  (including Alzheimer’s, LBD, FTD, vascular dementia, etc.)
